and IRS Form 8821 (Tax Information Authorization) allowing Lasiter to monitor the ERC

refund on behalf of our clients.

Upload Documents

Form 941’s – Employer's Quarterly Tax Returns for 2020 and 2021

  • Include any worksheets attached to the 941s


PPP Applications and Forgiveness, if applicable

  • Amount of any PPP funds received, and the date received
  • This can be a screenshot, bank statement, email, SBA, etc


Gross receipts broken out by quarter for 2019, 2020, and 2021

  • This can be a P&L statement or spreadsheet showing amounts. We just need to know how much money came into the company for each quarter.


Payroll report showing the gross wages for each employee broken out for each quarter of 2019, 2020 and 2021 (in CSV or Excel format).


Payroll report showing the gross wages for each employee for the period March 13th, 2020 – March 31st, 2020 (in CSV or Excel format)

  • This is the only portion of the 1st Quarter 2020 that qualifies.


Employer provided health insurance paid by the employer broken out by quarter for 2020 and 2021 (if applicable) This is only the portion paid by the Employer, not the Employee.
